Error code 5 is for unlocked HD. The Xbox is trying to boot the original dashboard with the original Bios...if the HD is unlocked it will give you that error code.
If you were to boot with a hacked bios running, or lock the HD to the mobo that error code will be gone.

Evolution X dash tools will do the trick if you have a dash installed, otherwise, you will have to use an autoinstaller, config magic...stuff like that.
But, if you flash your mod chip with a hacked bios, it won't matter if your HD is locked..then you can try installing some tools to help you lock it so when you launch the MS bios instead of your hacked bios all will be good.

Well, that's because there is either a problem with the chip's bios that you flashed to it (if you flashed it) or if it's a solderless chip it moved on you and needs tightened or realinged, or if you soldered it then you need to check your solder points.
I have had a few chips after soldering work fine for about seven hours or so, only to frag on me because of a bad solder point. Why this occurs is simple. If you have one point that is not soldered correctly, maybe there is a slight gap, it may be just good enough to get the machine to boot up fine. But, after a machine heats up things tend to expand, then as they cool, contract, so that creates movement in some areas. This just happened to me yesterday with an LT OPX chip. Quick solder method to the mobo on a 1.6. Worked great, I formatted and loaded three HDs and on the third HD suddenly the machine froze on me while launching an applications. Upon restart I got the three power cycle/frag. All I did was hit my solder points and now it's fine.
