Error 21 is a pretty generic error and I have fixed it in the past by the following.
1. Simply powering off and back on the XBOX
2. If you have the gamesaves on your XBOX just popping in the exploit game and redoing the mod
3. With a PC and Pimped
4. With Cold booting XBMC Spoof Disc Pluss, E22 Fix Disc or AID and redoing the mod

It also depends if you are chipped or softmodded. Since they don't sell chips at all anymore, I'm going to assume you're softmodded. If that is the case, then you will need to try one of SPPV's methods. If you're chipped, you can just pop in AID, reinstall the MS dash and you'll be on your way. _________________

Error 16 isnt an empty HDD, its dash boot failure, usualy caused by clock issue, in this case during his remod he most likely delted an xbe which will leave you hookered until replaced.

As long as it can boot a burned disc you should be able to fix it using AID or one of my boot discs. If it wont then yes its hot swap time, but thats come along ways since the old days. Ive put a lot of work into making an easy to use HDD build, mod, repair disc which pretty much does it all for you.
