Watch either my SID5 or softmod tutorial, it shows how to make an adapter and hook it up,
For best results use windows XP home SP3 based PC, Action Replay 1.42, either M$ brand or Madcatz memory card and one of the second generation black M$ 686 controllers. If after installing AR when you plug in your controller its not recognizing it, instead of letting it search for driver software click on browse and point it to the Datel folder on C drive in program files.

what version of AR are you using, mem card brand and please if you can post a pic of the controler
I use AR 1.42, Madcatz memcard and second gen black 686 M$ brand controller. I couldnt get the clear or original big ones to recognize, and I needed to manualy download and install the drvers with AR 1.3
Try opening device manager, the controller and mem card should show up with a question mark, right click and pick update driver, when the driver update window opens pick install from a specific location and try picking the driver folder in the AR folder in program files

Thats probably a big part of your problem, the third party stuff never seemed to work to well for modding. Ive never even heard of I&S memory cards
This is the controller ive found works best

Only other options are chipping it and hotswap, you cant FTP into the xbox till its been modded.
Unless you want to try making a new AR adaptor and go the USB route but you could run into the same problem of it not getting recognized
